Nexlizet is indicated for use in adults with heterozygous familial hypercholesterolemia (HeFH) or established atherosclerotic cardiovascular disease (ASCVD) who require additional LDL cholesterol lowering when diet and maximum tolerated statin therapy alone is inadequate.

Nexlizet combines two cholesterol-lowering agents: bempedoic acid inhibits ATP citrate lyase in the liver, reducing cholesterol synthesis, while ezetimibe inhibits the NPC1L1 transporter in the intestinal wall, blocking cholesterol absorption. Together they provide complementary LDL reduction.

Nexlizet may cause elevated liver enzymes and has been associated with tendon rupture risk. It is contraindicated in pregnancy. Common side effects include upper respiratory infections, muscle spasms, and back pain. Patients should maintain a cholesterol-lowering diet during treatment.
