Ofloxacin treats respiratory infections (such as pneumonia and bronchitis), urinary tract infections, skin and soft tissue infections, prostatitis, and sexually transmitted infections including gonorrhea and chlamydia. It is effective against both gram-positive and gram-negative bacteria.

Ofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination. By blocking these enzymes, it prevents bacteria from multiplying and spreading.

Common side effects include nausea, diarrhea, headache, and dizziness. Patients should avoid excessive sunlight exposure as photosensitivity reactions may occur. Fluoroquinolones may increase the risk of tendinitis and tendon rupture, particularly in older adults and those taking corticosteroids. Ofloxacin may also cause central nervous system effects including seizures and confusion.
