Published: July 26, 2026Updated: July 26, 2026

Ohtuvayre (ensifentrine) is a prescription inhaled medication used as maintenance treatment for chronic obstructive pulmonary disease (COPD) in adults. It is delivered via nebulizer and works as a dual phosphodiesterase 3 and 4 inhibitor, helping to reduce airway inflammation and improve breathing. This was the first COPD inhaler approved in decades that does not contain a corticosteroid.

Quick summary

Ohtuvayre is a nebulized COPD maintenance therapy that uses a novel dual PDE3/PDE4 inhibitor mechanism to improve breathing and reduce exacerbations in adults.

Ohtuvayre is indicated for the maintenance treatment of COPD in adults. It is not intended for acute bronchospasm relief and should not replace rescue inhalers.

Ohtuvayre contains ensifentrine, a dual phosphodiesterase 3 (PDE3) and phosphodiesterase 4 (PDE4) inhibitor. By inhibiting these enzymes, it increases intracellular cyclic AMP levels, which helps reduce inflammation, relax airway smooth muscle, and improve lung function.

Ohtuvayre is administered twice daily via nebulizer. Common side effects may include back pain, diarrhea, and urinary tract infections. Patients should inform their healthcare provider of all medications and medical conditions before starting treatment. Effectiveness and safety in pediatric patients have not been established.
