Treatment of active mild to moderate ulcerative colitis and maintenance of remission in patients with chronic ulcerative colitis.

Olsalazine is a prodrug that remains inactive until it reaches the colon, where resident bacteria cleave the azo bond to release 5-aminosalicylic acid (5-ASA/mesalazine). The 5-ASA then exerts a topical anti-inflammatory effect directly on colonic epithelial cells, reducing inflammation in the intestinal mucosa.

Diarrhea is a common side effect (occurring in approximately 10-20% of patients). Should be used with caution in patients with renal impairment and avoided in patients with known hypersensitivity to salicylates. May also cause photosensitivity reactions. Regular monitoring of renal function may be recommended during prolonged therapy.
