Opsynvi is indicated for the treatment of pulmonary arterial hypertension (PAH) in adults, including those with certain types of PAH associated with connective tissue disease, to improve exercise capacity and delay clinical worsening.

Opsynvi combines two active ingredients: macitentan (an endothelin receptor antagonist) and tadalafil (a phosphodiesterase type 5 inhibitor). These agents work through complementary mechanisms to reduce pulmonary vascular resistance by blocking endothelin and enhancing nitric oxide pathways, resulting in vasodilation of the pulmonary arteries.

Opsynvi is contraindicated in pregnancy and should not be used in patients with hypersensitivity to any components. Regular monitoring of liver function and blood counts is recommended. Common side effects may include headache, anemia, and peripheral edema. Patients should avoid concurrent use with certain medications including nitrates and strong CYP3A4 inhibitors.
