Oseni is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus. It is typically prescribed when treatment with either agent alone is insufficient.
The medication combines two mechanisms: alogliptin inhibits the DPP-4 enzyme, which increases levels of incretin hormones (GLP-1 and GIP) to stimulate insulin release and reduce glucose production; pioglitazone activates PPAR-gamma receptors to increase insulin sensitivity in fat, muscle, and liver cells.
Oseni is not for use in type 1 diabetes or diabetic ketoacidosis. Common side effects may include upper respiratory tract infection, headache, and weight gain. Pioglitazone component carries risks of fluid retention, heart failure, and bone fractures. It is contraindicated in patients with established heart failure (NYHA Class III-IV) and should not be used in patients with active bladder cancer or unexplained hematuria.
