Pacritinib
Pacritinib is an oral kinase inhibitor used primarily for the treatment of myelofibrosis, a rare bone marrow disorder characterized by abnormal blood cell production and fibrosis. It works by targeting multiple kinases including JAK2, FLT3, and IRAK1, which are involved in abnormal cellular signaling that drives the disease. This medication is particularly considered for patients who have difficulty tolerating other JAK inhibitors due to low platelet counts.

Treatment of myelofibrosis, including primary myelofibrosis, post-polycythemia vera myelofibrosis, and post-essential thrombocythemia myelofibrosis. Often considered for patients with cytopenias (low blood cell counts) who may not tolerate other JAK inhibitor therapies well.
Pacritinib inhibits multiple kinases including wild type and mutant JAK2V617F, FLT3, and IRAK1. These kinases play key roles in cytokine and growth factor signaling pathways that drive the abnormal blood cell production and bone marrow fibrosis seen in myelofibrosis.
As an antineoplastic agent, pacritinib carries risks of myelosuppression, infections, and other adverse effects requiring monitoring. Drug interactions may occur with CYP3A4 inhibitors or inducers. Use is typically reserved for patients under specialist supervision in hematology/oncology settings.
