Padcev is indicated for the treatment of adults with locally advanced or metastatic urothelial cancer (bladder cancer, ureter cancer, or cancer of the renal pelvis) who have previously received a programmed death receptor-1 (PD-1) or programmed death-ligand 1 (PD-L1) inhibitor and a platinum-containing chemotherapy regimen.

Padcev is an antibody-drug conjugate (ADC) consisting of a monoclonal antibody that binds to Nectin-4, a cell adhesion protein that is highly expressed on urothelial cancer cells. Once bound, the conjugate is internalized and releases the cytotoxic agent monomethyl auristatin E (MMAE), which disrupts the microtubule network within the cancer cell, leading to cell death.

Padcev may cause serious side effects including skin reactions, hyperglycemia, peripheral neuropathy, ocular disorders, and infusion site extravasation. It is contraindicated in patients with known severe hypersensitivity to the drug. Treatment should be administered by healthcare professionals experienced in the use of anticancer agents. Pregnant women should not receive Padcev due to potential harm to the fetus.
