Palynziq is indicated for adults with phenylketonuria (PKU) who have inadequate blood phenylalanine control (typically >600 μmol/L) despite dietary management or other treatments. It is specifically approved for patients 18 years and older.

Palynziq contains pegvaliase-pqpz, a recombinant PEGylated phenylalanine ammonia lyase (PAL) enzyme. This enzyme converts phenylalanine into trans-cinnamic acid and ammonia, effectively metabolizing and reducing circulating phenylalanine levels in the blood.

Palynziq is administered via subcutaneous injection. Common side effects include injection site reactions, arthralgia, hypersensitivity reactions, and headache. Patients require monitoring of blood phenylalanine levels and may need to continue dietary phenylalanine restriction. Treatment initiation requires a slow dose escalation protocol and carries a risk of severe allergic reactions, including anaphylaxis.
