Perindopril

Published: July 26, 2026Updated: July 26, 2026

Perindopril is an ACE inhibitor (angiotensin-converting enzyme inhibitor) used to treat hypertension, heart failure, and reduce cardiovascular risk. It is a pro-drug that converts to its active form, perindoprilat, which works by blocking ACE, an enzyme responsible for producing angiotensin II—a substance that constricts blood vessels and raises blood pressure.

Quick summary

Perindopril is an ACE inhibitor used to treat high blood pressure and heart conditions by relaxing blood vessels.

Pharmacodynamic mechanism

Perindopril is used to treat essential hypertension, stable coronary artery disease, and heart failure. It may also be prescribed to reduce the risk of cardiovascular events such as stroke and heart attack in certain patients.

Perindopril is converted to perindoprilat, its active metabolite, which inhibits ACE (angiotensin-converting enzyme). This reduces the production of angiotensin II, a potent vasoconstrictor, leading to relaxation and widening of blood vessels, which lowers blood pressure.

Perindopril should not be used during pregnancy (especially second and third trimesters) as it may harm the fetus. Common side effects include dry cough, dizziness, and elevated potassium levels. It is often used in combination with other antihypertensive agents, particularly thiazide diuretics.
