Pimecrolimus is FDA-approved for the treatment of mild to moderate atopic dermatitis (eczema) in adults and children 2 years of age and older. It is particularly useful for sensitive skin areas such as the face, neck, and skin folds where topical corticosteroids may be less ideal.
Pimecrolimus is a calcineurin inhibitor that works by binding to a protein called macrolophilin-12 (FKBP-12), which inhibits calcineurin. This prevents the activation of T-cells and blocks the release of inflammatory cytokines, thereby reducing skin inflammation and associated symptoms like itching and redness.
Pimecrolimus carries a FDA black box warning due to a potential increased risk of lymphoma and skin cancer with its use. It should be used for short-term treatment only and not continuously on a long-term basis. Common side effects include skin burning, warmth, or itching at the application site. Patients should avoid exposure to natural or artificial sunlight and should not be used on malignant or potentially malignant skin conditions.
