Pirtobrutinib
Pirtobrutinib is a targeted cancer therapy classified as a noncovalent (reversible) inhibitor of Bruton's tyrosine kinase (BTK). It is used to treat certain B-cell blood cancers by blocking BTK signaling, which B-cells need for growth, survival, and movement. This medication is specifically indicated for adults with mantle cell lymphoma (MCL) and chronic lymphocytic leukemia (CLL) who have received prior treatments.

Pirtobrutinib is indicated for the treatment of adult patients with mantle cell lymphoma (MCL) and chronic lymphocytic leukemia (CLL)/small lymphocytic lymphoma (SLL) who have received at least two prior lines of therapy, including a BTK inhibitor and a BCL-2 inhibitor.
Pirtobrutinib is a small molecule that noncovalently (reversibly) inhibits BTK, a signaling protein in B-cells. By blocking BTK, it prevents activation of pathways necessary for B-cell proliferation, survival, trafficking, and chemotaxis, leading to reduced cancer cell growth.
Common side effects may include fatigue, musculoskeletal pain, and bruising. Patients should be monitored for infections, bleeding events, and cardiac arrhythmias. As a noncovalent inhibitor, pirtobrutinib may maintain activity in patients who have developed resistance to covalent BTK inhibitors. Pregnancy should be avoided during treatment due to potential harm to the fetus.
