Pomalidomide is indicated for the treatment of multiple myeloma in patients who have received at least two prior therapies, including lenalidomide and a proteasome inhibitor, in combination with dexamethasone.

Pomalidomide binds to cereblon, a component of the Cullin RING E3 ubiquitin ligase complex, leading to the degradation of specific transcription factors and resulting in immunomodulatory effects, antiangiogenic activity, and direct antineoplastic effects.

Pomalidomide is a thalidomide analogue with significant teratogenic risk and requires enrollment in a restricted distribution program (REMS). Complete blood counts should be monitored regularly due to risks of neutropenia and thrombocytopenia. Women of childbearing potential must use effective contraception during treatment.
