Probenecid
Orlynvah is a combination antibacterial medication containing sulopenem etzadroxil and probenecid, used to treat complicated urinary tract infections. The probenecid component works by inhibiting OAT3-mediated renal tubular transport, which slows the clearance of sulopenem from the body and increases its plasma concentrations, allowing for enhanced antibacterial effect.
A combination antibiotic for complicated urinary tract infections that uses probenecid to boost sulopenem effectiveness by reducing its renal clearance.

Orlynvah is indicated for the treatment of complicated urinary tract infections (cUTI) in adults, including pyelonephritis, caused by susceptible bacteria.
Sulopenem etzadroxil provides antibacterial activity against susceptible organisms, while probenecid inhibits the OAT3 transporter in renal tubules, reducing the renal clearance of sulopenem and increasing its plasma concentrations to maintain therapeutic levels.
This is an intravenous (IV) medication. Patients should be monitored for allergic reactions, C. difficile-associated diarrhea, and potential drug interactions. Probenecid may interact with other drugs eliminated via renal tubular secretion. Not recommended for patients with severe renal impairment.
