Pyrazinamide is indicated for the treatment of active pulmonary and extrapulmonary tuberculosis as part of a multi-drug regimen. It is particularly used during the intensive phase of TB therapy, often in combination with isoniazid, rifampin, and/or ethambutol.

Pyrazinamide is a prodrug that is converted by Mycobacterium tuberculosis to its active form, pyrazinoic acid. The exact mechanism is not fully understood but is believed to involve disruption of bacterial fatty acid synthesis, energy production, and membrane transport function, leading to inhibition of bacterial growth.

Pyrazinamide can cause hepatotoxicity and requires regular monitoring of liver function tests. Hyperuricemia is a common side effect due to inhibition of renal uric acid excretion. It is generally avoided in patients with severe hepatic impairment and should be used with caution during pregnancy.
