Treatment of acute diarrhea in adults and children. It is used as part of oral rehydration therapy to reduce the duration and severity of diarrheal episodes, particularly in cases of acute gastroenteritis.

Racecadotril is a pro-drug that inhibits enkephalinase in the intestinal mucosa. This enzyme normally degrades enkephalins (endogenous opioid peptides). By preventing this breakdown, racecadotril increases enkephalin levels, which inhibits intestinal secretion of water and electrolytes while preserving normal intestinal motility.

Racecadotril is generally well-tolerated with a good safety profile. It does not cause constipation like typical opioid antidiarrheals. Treatment should begin at the first sign of diarrhea with a loading dose followed by maintenance doses. Adequate fluid and electrolyte replacement remains essential. Not typically used for chronic diarrhea or antibiotic-associated diarrhea. Common side effects may include headache and skin reactions.
