Recorlev is indicated for the treatment of endogenous hypercortisolism (Cushing's syndrome) in adult patients with either persistent or recurrent disease who are not candidates for surgery or have not responded adequately to surgery.

Recorlev (levoketoconazole) inhibits enzymes involved in cortisol synthesis, primarily cytochrome P450 11B1 (CYP11B1), thereby reducing the production of cortisol by the adrenal glands.

Recorlev carries a boxed warning for hepatotoxicity and requires regular liver function monitoring. It should only be prescribed by healthcare providers experienced in treating Cushing's syndrome. Common side effects may include nausea, hypocortisolism, and fatigue. Patients should be monitored for signs of adrenal insufficiency during treatment.
