Published: August 9, 2026Updated: August 9, 2026

Rectiv is a topical brand-name medication containing nitroglycerin that is applied rectally to treat pain associated with chronic anal fissures. It works by releasing nitric oxide, which causes smooth muscle relaxation in the anal region, reducing sphincter spasms and allowing fissures to heal. This medication is typically used when conservative treatments have not provided adequate relief.

Rectiv is indicated for the treatment of moderate to severe pain associated with chronic anal fissures (tears in the anal lining lasting 6 weeks or more) that have not responded to conservative treatments like dietary fiber supplements and stool softeners.

Rectiv contains nitroglycerin (0.4%), a nitrate that releases nitric oxide when absorbed. Nitric oxide activates guanylate cyclase in smooth muscle cells, increasing cyclic GMP levels, which leads to smooth muscle relaxation. In the anal region, this relaxes the internal anal sphincter, reducing spasm, decreasing pressure, and improving blood flow to promote fissure healing.

Rectiv is for rectal use only and should be applied with a finger covering or applicator as directed by a healthcare provider. Common side effects may include headache, dizziness, and rectal discomfort. Use with caution in patients taking phosphodiesterase inhibitors (e.g., sildenafil), as severe hypotension may occur. Avoid use in severe anemia, recent head trauma, or increased intracranial pressure. Treatment courses are typically limited to 3 weeks.
