Treatment of acute myocardial infarction (heart attack) to reduce mortality and preserve heart muscle function when administered within 12 hours of symptom onset.

Reteplase is a genetically engineered tissue plasminogen activator (tPA) that converts plasminogen to plasmin. Plasmin then breaks down fibrin, the structural framework of blood clots, restoring blood flow through occluded coronary arteries.

Must be given as early as possible for maximum benefit. Carries significant bleeding risk, especially intracranial hemorrhage. Contraindicated in active bleeding, recent surgery, history of stroke, and uncontrolled hypertension. Requires careful patient selection and monitoring.
