Rivaroxaban (Xarelto): A Patient Guide

Published: July 22, 2026Updated: July 22, 2026

Rivaroxaban is a prescription anticoagulant, or “blood thinner,” that reduces the risk of blood clots forming. It is used to prevent stroke in people with atrial fibrillation and to treat or prevent deep vein thrombosis and pulmonary embolism in certain patients.

Quick summary

Rivaroxaban can cause serious bleeding, so take it exactly as prescribed and do not stop it without medical advice. Contact a clinician promptly for signs of bleeding or a serious reaction.

Overdose information

Important overdose information
Toxic dose: Doses greater than 50 times the therapeutic dose (≥1000mg) may lead to severe bleeding complications
Chronic overdose: Prolonged supratherapeutic dosing or accumulation leading to supratherapeutic anticoagulation and bleeding risk
Immediate actions
Activated charcoal within 8 hours of ingestion if no active bleeding, hospital observation for bleeding monitoring
Antidote
Andexanet alfa (approved in US) or Prothrombin complex concentrates(Andexanet alfa is a specific reversal agent; PCCs may be considered for life-threatening bleeding)
