Treatment of chronic hepatitis C virus (HCV) infection in adults. Used in combination with other antiviral agents (such as velpatasvir and ledipasvir) for multiple HCV genotypes. Often prescribed for patients who have not responded to previous hepatitis C treatments.

Sofosbuvir is a nucleotide analogue prodrug that is metabolized intracellularly to its active form (GS-461203). It acts as a faulty substrate for the HCV NS5B RNA-dependent RNA polymerase, causing chain termination and inhibiting viral RNA replication. This direct action on the virus prevents it from multiplying and spreading.

Must be used in combination with other antiviral medications as monotherapy is not recommended. Contraindicated in pregnancy and should not be used with certain drugs that may reduce efficacy. Ribavirin combination is contraindicated in pregnant women and male partners of pregnant women. Common side effects may include fatigue and headache. Regular monitoring of liver function is recommended during treatment. Should only be prescribed by qualified healthcare providers experienced in treating hepatitis C.
