Sonidegib is indicated for the treatment of adult patients with locally advanced or metastatic basal cell carcinoma (BCC) that has recurred following surgery or radiation therapy, or for patients who are not candidates for surgery or radiation.
Sonidegib is a selective Smoothened (SMO) inhibitor that binds to SMO, a transmembrane protein critical for Hedgehog signal transduction. By inhibiting SMO, it blocks downstream signaling of the Hedgehog pathway, which is abnormally activated in certain cancers including basal cell carcinoma.
Sonidegib carries a Boxed Warning for embryo-fetal toxicity and embryolethality. Pregnancy must be avoided during treatment and for at least 20 months after the last dose. Blood and sperm donation is prohibited during and after treatment. Common adverse reactions include muscle spasms, alopecia, dysgeusia, fatigue, nausea, musculoskeletal pain, and diarrhea.
