Sotatercept

Published: July 26, 2026Updated: July 26, 2026

Sotatercept is a recombinant activin receptor type IIA-Fc fusion protein used to treat pulmonary arterial hypertension (PAH). It works by binding to activin A and other TGF-β superfamily ligands, inhibiting activin signaling to improve the balance between pro-proliferative and anti-proliferative pathways in pulmonary vasculature, thereby reducing pulmonary vascular resistance.

Quick summary

Sotatercept is an injectable activin signaling inhibitor used for pulmonary arterial hypertension that works by binding TGF-β superfamily ligands to improve vascular balance.

Pulmonary arterial hypertension (PAH) in adults with WHO Group 1 PH as add-on therapy to background PAH treatments to improve exercise capacity.

Sotatercept is a fusion protein consisting of the extracellular domain of activin receptor type IIA fused to an Fc fragment. It acts as a ligand trap, binding activin A and other TGF-β superfamily ligands to inhibit activin signaling. This restores the balance between pro-proliferative (ActRIIA) and anti-proliferative pathways, reducing pulmonary vascular remodeling and resistance.

Administered as a subcutaneous injection, typically every 3 weeks. May cause thrombocytopenia and requires regular blood monitoring. Contraindicated in pregnancy due to potential fetal harm. Women of reproductive potential should use effective contraception during treatment.
