Bexarotene
Targretin (bexarotene) is a retinoid medication used to treat certain types of skin lymphoma, specifically cutaneous T-cell lymphoma (CTCL) in patients who have not responded to at least one prior systemic therapy. Targretin is FDA-approved for the treatment of cutaneous T-cell lymphoma (CTCL) in patients with refractory or persistent disease after at least one prior systemic therapy. The topical gel formulation is used for early-stage skin lesions, while the oral capsule may be used for more advanced disease.
Targretin is a synthetic retinoid that selectively binds to and activates retinoid X receptors (RXRs). By activating these nuclear receptors, it influences gene expression involved in cell differentiation, proliferation, and apoptosis, helping to control the abnormal growth of malignant T-cells in the skin.
Common side effects include hyperlipidemia (elevated cholesterol and triglycerides), hypothyroidism, headache, and skin irritation at application sites. Regular blood monitoring for lipid levels and thyroid function is required. Targretin is contraindicated in pregnancy due to teratogenic effects. Grapefruit juice should be avoided as it may increase drug levels.
