Tarpeyo is indicated to reduce proteinuria (excess protein in urine) in adults with primary immunoglobulin A nephropathy (IgAN), a kidney disease where abnormal antibodies damage kidney filters, in patients at risk of progressive kidney function loss.

Tarpeyo delivers budesonide directly to the distal ileum (lower small intestine), where it acts locally on gut-associated lymphoid tissue to modulate the immune response that produces harmful IgA antibodies, thereby reducing kidney inflammation and protein loss.

Tarpeyo is taken as one capsule (9 mg) once daily with food. Common side effects may include hypertension, peripheral edema, muscle spasms, and acne. It is contraindicated in patients with severe hepatic impairment and may interact with CYP3A4 inhibitors. Regular monitoring of kidney function and blood pressure is recommended during treatment.
