Tenecteplase is used to treat acute ST-elevation myocardial infarction (STEMI) when percutaneous coronary intervention is not available within recommended timeframes. It may also be used for acute massive pulmonary embolism with hemodynamic instability.

Tenecteplase is a genetically modified form of human tissue plasminogen activator that binds specifically to fibrin within blood clots. Once bound to fibrin, it converts plasminogen to plasmin, which then breaks down the fibrin structure and dissolves the clot.

This medication carries significant bleeding risk and is contraindicated in patients with active internal bleeding, recent surgery or trauma, history of hemorrhagic stroke, or uncontrolled hypertension. It should be administered as soon as possible after symptom onset for maximum benefit. Anticoagulant and antiplatelet medications may increase bleeding risk when used with tenecteplase.
