TNKase is indicated for the treatment of acute ST-elevation myocardial infarction (STEMI) to reduce mortality and cardiovascular complications. It is typically used when prompt percutaneous coronary intervention (PCI) is not available within appropriate timeframes.

Tenecteplase is a recombinant tissue plasminogen activator that binds to fibrin within blood clots and converts plasminogen to plasmin. This plasmin then breaks down the fibrin meshwork of the clot, restoring blood flow through the occluded coronary artery.

TNKase carries a significant risk of bleeding, including potentially life-threatening intracranial hemorrhage. It is contraindicated in patients with active internal bleeding, recent surgery, history of stroke, or uncontrolled hypertension. The medication is dosed based on patient body weight and administered as a single IV bolus over 5 seconds.
