Tracleer is primarily used to treat pulmonary arterial hypertension (PAH) to improve exercise capacity and symptoms, and to reduce the number of new digital (finger) ulcers in patients with systemic sclerosis (scleroderma).

Tracleer is an endothelin receptor antagonist that blocks endothelin-1 (ET-1), a potent vasoconstrictor and pro-fibrotic substance. By blocking ET-1 receptors, it allows blood vessels to relax and widens them, reducing pulmonary blood pressure and improving blood flow.

Tracleer is a specialty medication available only through restricted distribution programs due to potential liver toxicity and teratogenic effects. Regular liver function monitoring and monthly pregnancy tests are required. It may cause fluid retention, anemia, and interacts with multiple medications including hormonal contraceptives and certain antifungal drugs.
