Voclosporin (Lupkynis) is a prescription immunosuppressant medication classified as a calcineurin inhibitor. Voclosporin is indicated for the treatment of adult patients with active lupus nephritis (class III-V) to reduce proteinuria and improve renal outcomes in combination with other immunosuppressive therapy.

As a calcineurin inhibitor, voclosporin blocks the calcineurin pathway, which is involved in T-cell activation. This suppresses lymphocyte-mediated immune responses that contribute to kidney inflammation in lupus nephritis.

Common side effects include decreased glomerular filtration rate, hypertension, diarrhea, headache, and anemia. Patients require regular monitoring of blood pressure, kidney function, and lipid levels. Use is contraindicated in patients with severe renal impairment or taking strong CYP3A4 inhibitors. May increase infection risk including viral reactivation.
