Published: July 26, 2026Updated: July 26, 2026

Vorasidenib is an oral targeted therapy that inhibits IDH1 and IDH2 enzymes, which are mutated in certain brain tumors. It is specifically approved for the treatment of grade 2 glioma in adults who have a susceptible IDH1 or IDH2 mutation and require additional surgery after initial biopsy or partial resection. This precision medicine approach targets the specific genetic drivers of certain brain tumors.

Treatment of grade 2 glioma in adults with susceptible IDH1 or IDH2 mutations who require additional surgery following biopsy or partial resection. Diagnosis of IDH mutation status should be confirmed using an FDA-approved companion diagnostic test prior to treatment initiation.

Vorasidenib is a small molecule inhibitor that targets and blocks isocitrate dehydrogenase-1 (IDH1) and isocitrate dehydrogenase-2 (IDH2) enzymes. In tumors with IDH mutations, these enzymes produce an oncometabolite (2-hydroxyglutarate) that promotes tumor growth and progression. By inhibiting these mutated enzymes, vorasidenib reduces levels of this oncometabolite and may slow tumor growth.

Common adverse reactions include fatigue, headache, COVID-19 infection, musculoskeletal pain, diarrhea, nausea, and seizure. Hepatotoxicity with elevated liver enzymes may occur and requires regular monitoring. May cause fetal harm; pregnancy testing recommended before starting treatment. Should be taken as directed with or without food; tablets should be swallowed whole without splitting or crushing.
