Xofigo (radium Ra 223 dichloride) is a targeted radiopharmaceutical agent used to treat men with castration-resistant prostate cancer that has spread to the bones. It works by mimicking calcium and binding to areas of increased bone turnover in metastatic lesions, where it delivers localized alpha radiation to destroy cancer cells while minimizing damage to surrounding healthy tissue.

Treatment of adult men with castration-resistant prostate cancer (CRPC) that has spread to the bones with symptomatic bone metastases. It is not indicated for patients with visceral metastases or for use in combination with other systemic therapies.

Xofigo is a radium-223 dichloride that acts as a calcium mimetic. It selectively targets areas of osteoblastic (new bone) activity common in bone metastases. Once incorporated into bone matrix, it emits high-energy alpha particles that cause double-strand DNA breaks in nearby cancer cells, leading to cell death.

Administered as an intravenous injection once monthly for 6 cycles. Common side effects include bone pain, nausea, diarrhea, and myelosuppression (anemia, thrombocytopenia, neutropenia). Requires handling by qualified healthcare personnel with appropriate radiation safety measures. Pregnancy category X - contraindicated in women. Patients should maintain adequate contraception during and after treatment.
