Ziextenzo is a prescription medication containing the active ingredient pegfilgrastim, a long-acting granulocyte colony-stimulating factor (G-CSF). Ziextenzo is indicated to reduce the likelihood of infection in patients with non-myeloid malignancies undergoing myelosuppressive chemotherapy. It is also used to increase survival in patients exposed to myelosuppressive doses of radiation.

Ziextenzo is a biosimilar to Neulasta containing pegfilgrastim, which binds to granulocyte colony-stimulating factor (G-CSF) receptors on hematopoietic progenitor cells in the bone marrow. This binding stimulates the production, proliferation, and activation of neutrophils, thereby reducing the risk of febrile neutropenia.

Ziextenzo is administered as a single subcutaneous injection once per chemotherapy cycle, typically at least 24 hours after chemotherapy. Common side effects may include bone pain and injection site reactions. Serious potential adverse effects include splenic rupture, acute respiratory distress syndrome, and allergic reactions. Patients with known hypersensitivity to pegfilgrastim or filgrastim products should not use this medication.
