Zilbrysq is indicated for the treatment of generalized myasthenia gravis (gMG) in adult patients who are anti-acetylcholine receptor (AChR) antibody positive. It is not indicated for the treatment of ocular myasthenia gravis.
Zilbrysq is a complement C5 inhibitor that works by binding to complement protein C5, preventing its cleavage into C5a and C5b. This inhibits the formation of the membrane attack complex (MAC), reducing immune-mediated destruction of the neuromuscular junction in myasthenia gravis.
Zilbrysq carries a boxed warning for serious meningococcal infections due to complement inhibition. Patients should be vaccinated against meningococcal infections before starting treatment and monitored closely for signs of infection. Common side effects may include injection site reactions and upper respiratory infections. It is administered as a once-daily subcutaneous injection.
