Zolbetuximab

Published: July 26, 2026Updated: July 26, 2026

Zolbetuximab is a monoclonal antibody that targets claudin 18.2 (CLDN18.2), a protein expressed on certain cancer cells, particularly gastric and gastroesophageal cancers. It works by binding to CLDN18.2-positive cells and triggering immune-mediated cell destruction through antibody-dependent cellular cytotoxicity (ADCC) and complement-dependent cytotoxicity (CDC). This medication is used in combination with chemotherapy for the treatment of adults with locally advanced unresectable or metastatic HER2-negative gastric or gastroesophageal junction adenocarcinoma whose tumors express CLDN18.2.

Quick summary

Zolbetuximab is a targeted antibody therapy for CLDN18.2-positive gastric and gastroesophageal cancers that works by recruiting the immune system to destroy cancer cells.

Treatment of locally advanced unresectable or metastatic HER2-negative gastric or gastroesophageal junction adenocarcinoma expressing claudin 18.2 (CLDN18.2) in combination with chemotherapy in adult patients

Zolbetuximab is a claudin 18.2-directed cytolytic antibody that binds to CLDN18.2-positive cells and triggers immune-mediated cell death through two mechanisms: antibody-dependent cellular cytotoxicity (ADCC), where immune cells are recruited to attack the cancer cells, and complement-dependent cytotoxicity (CDC), where the complement system is activated to destroy the target cells.

Administered intravenously; should only be used in patients whose tumors have confirmed CLDN18.2 expression. Common adverse reactions may include nausea, vomiting, decreased appetite, and infusion-related reactions. May interact with other medications; healthcare providers should review full medication history. Pregnancy should be avoided during treatment due to potential harm to the fetus.
