Zontivity is indicated to reduce the risk of thrombotic cardiovascular events in adult patients with a history of heart attack (myocardial infarction) or peripheral arterial disease (PAD). It is not approved for use in patients who have had a stroke or transient ischemic attack (TIA).

Zontivity contains the active ingredient vorapaxar, which is a protease-activated receptor-1 (PAR-1) antagonist. It works by blocking PAR-1 receptors on platelets, preventing thrombin-mediated platelet activation and aggregation, thereby reducing the formation of blood clots.

Zontivity carries a boxed warning for bleeding risk, including serious and sometimes fatal bleeding. It is contraindicated in patients with active pathological bleeding or a history of stroke, TIA, or intracranial hemorrhage. Bleeding risk is increased when combined with other anticoagulants or antiplatelet agents. Patients should not discontinue the medication without consulting their healthcare provider.
