Recovery phases

Week 1Acute Protection

Pain at lateral epicondyle and medial epicondyle ≤3/10 during ADLs,No aggravating valgus stress to medial epicondyle,Ulnar nerve neurosensory status stable (no paresthesia worsening),Full pain-free passive elbow extension-flexion arc (0-145°),No joint line tenderness to palpation over humeroulnar cartilage,Isometric forearm extensor/flexor contraction pain ≤4/10

→ Advance to: Mobility & Early Strengthening

Week 5Mobility & Early Strengthening

Pain at common extensor origin (lateral epicondyle) ≤2/10 with resistive wrist extension,Pain at common flexor origin (medial epicondyle) ≤2/10 with resistive wrist flexion,Full pain-free pronation-supination range of motion,No ulnar nerve provocation with elbow flexion >90°,Grip strength ≥70% of contralateral side (Jamar dynamometer),No crepitus with elbow joint movement (cartilage integrity)

→ Advance to: Progressive Strengthening

Week 9Progressive Strengthening

Pain-free eccentric loading of extensor carpi radialis brevis at lateral epicondyle,Pain-free eccentric loading of flexor-pronator mass at medial epicondyle,Ulnar nerve Tinel sign negative or unchanged from baseline,Full elbow joint accessory motion (humero-ulnar),Single-arm plank holding ≥30 seconds,Wrist extension strength ≥85% contralateral side

→ Advance to: Return to Function

Week 15Return to Function

Return to sport/work activities without elbow pain recurrence,Full pain-free power grip and sustained grip (lateral epicondyle stress),Pain-free valgus stress testing (medial epicondyle stability),No ulnar nerve symptoms with overhead activities,Functional movement pattern assessment passed (throwing, serving, lifting),Patient-reported outcome measure score meets MCID threshold

→ Advance to: Maintenance & Discharge

Return to activity milestones

Pain-free basic activities of daily living

Week 6

Pain ≤ 3/10 during functional reaching and lifting activities

Light functional activities and conditioning

Week 9

No increase in pain at 48-hour follow-up after exercise progression

Moderate activity and sport-specific conditioning

Week 12

Elbow flexion and extension range adequate for ADL completion

Full return to sport, occupation, and recreation

Week 20

Grip strength ≥ 50% of unaffected side on dynamometry

Precautions
