Recovery phases
Pain at rest < 3/10 on VAS scale,Weight-bearing tolerance achieved without increased groin or lateral hip pain,Hip flexion ROM 70-90 degrees without impingement symptoms,Ability to perform supine hip_log_roll without exacerbating labral symptoms,Greater trochanteric bursa tenderness reduced by 50% on palpation,Independent ambulation with assistive device (if needed) at household distances
→ Advance to: Subacute Phase
Hip flexion ROM 100-110 degrees achieved without anterior hip clicking,Hip internal rotation at 90 degrees flexion > 20 degrees without posterior hip pain,Pain-free active hip flexion in hook-lying position without labral catch,Gluteus medius strength 4/5 on manual muscle testing in side-lying abduction,Iliopsoas tendon flexibility improved allowing prone knee flexion to 90 degrees,Single leg stance time > 10 seconds without Trendelenburg sign
→ Advance to: Intermediate Phase
Hip abduction strength 4+/5 with resistance without greater trochanteric pain,Hip internal and external rotation against resistance without groin pain,Active straight leg raise without hip flexor tendon strain,Squat to 45 degrees knee flexion without anterior hip impingement,Clamshell exercise with resistance without piriformis or ischiofemoral pain,Walking gait pattern normalized without antalgic components
→ Advance to: Advanced Strengthening Phase
Hip extension strength 5/5 on prone manual muscle testing,Single leg squat to 60 degrees knee flexion without knee valgus or hip adduction,Hip adductor strength 4+/5 without adductor tendon strain,Progressive plyometric tolerance without impact-related hip pain,Ischiofemoral ligament flexibility allowing prone knee flexion beyond neutral,Functional movement pattern screening score > 18/21 on FMS hip mobility
→ Advance to: Return to Activity Phase
Full pain-free hip ROM in all planes matching contralateral side,Single leg hop for distance > 90% of contralateral limb,Agility testing at 75% speed without hip symptoms,Sport-specific movement patterns completed without mechanical hip symptoms,Hip Outcome Score ADL subscale > 85%,iliopsoas and hip flexor endurance test > 90 seconds hold
→ Advance to: Sport/Activity Re-entry
Full return to sport-specific training without restrictions,iHOT-12 score > 80/100 indicating minimal disability,Harris Hip Score > 90 indicating excellent functional outcome,Complete return to high-impact activities without flare-ups,Hip abduction and adduction strength within 10% of contralateral side,Patient subjective report of full recovery and confidence in hip function
→ Advance to: Discharge/Monitoring
Return to activity milestones
Pain-free basic activities of daily living
Week 7Ankle dorsiflexion adequate for normalised gait pattern over heel
Light functional activities and conditioning
Week 10Weight-bearing tolerance increased by ≥ 25% from previous baseline
Moderate activity and sport-specific conditioning
Week 14Able to ascend and descend 4 stairs with reciprocal pattern, no rail
Full return to sport, occupation, and recreation
Week 24Weight-bearing tolerance increased by ≥ 25% from previous baseline

- warm-up-before
Contraindications
- wound dehiscence
- unstable-fixation
- acute-inflammation
Pain management
- Ice application to greater trochanter and anterior hip for 15-20 minutes post-activity
- Activity modification to avoid hip flexion beyond 90 degrees and deep squatting
- Non-steroidal anti-inflammatory medication as prescribed for tendon/bursa inflammation
- Positional relief using pillow between knees in sidelying for bursa decompression
- Gentle iliopsoas and hip flexor stretching in pain-free range
