Advanced Shoulder Tendon Repair Rehabilitation
A 10-week advanced program focused on rehabilitating the shoulder after tendon repair, targeting the rotator cuff, labrum, AC joint, and biceps tendon using resistance bands and hand exercisers.
Professional rehabilitation program designed for effective recovery and pain management.

Recovery phases
Pain level ≤3/10 at rest and with passive glenohumeral movements,Surgical incision healed without signs of infection,Passive glenohumeral flexion to 90° achieved without pain,Scapular kinematics assessed as stable with minimal dyskinesis,No active external rotation or abduction against resistance
→ Advance to: Early Motion Phase
Pain level ≤2/10 with passive ROM exercises,Passive glenohumeral flexion to 120° achieved,External rotation to 30° in scapular plane achieved,Scapular elevation and retraction control demonstrated in prone,No exacerbation of biceps tendon tenderness at anterior shoulder
→ Advance to: Early Motion Phase
Passive glenohumeral flexion to 145° achieved,External rotation in scapular plane to 45° achieved,Active-assisted ROM demonstrating proper glenohumeral rhythm,No pain with rotator cuff isometric contractions at low effort,Scapular stabilization in side-lying with proper periscapular muscle activation
→ Advance to: Progressive Motion Phase
Passive glenohumeral flexion to 170° achieved,External rotation to 60° at 90° abduction,Internal rotation to 70° in scapular plane,Active glenohumeral ROM with minimal compensatory scapular elevation,Isometric rotator cuff contractions tolerated without pain
→ Advance to: Progressive Motion Phase
Full passive glenohumeral ROM achieved in all planes,Glenohumeral translation assessed as stable on anterior/posterior drawer,Active range of motion matching passive ROM within 10°,No AC joint tenderness with cross-body adduction,Scapular control during active arm elevation against gravity
→ Advance to: Strengthening Phase
Full active ROM achieved with proper glenohumeral-scapular rhythm,Rotator cuff strength grade 3/5 in internal/external rotation,No labral provocation with active compression test,Scapular stabilization strength adequate in prone protraction,Biceps tendon resistance testing without anterior shoulder pain
→ Advance to: Strengthening Phase
Rotator cuff strength grade 4-/5 in external rotation at 90° abduction,Scapular strengthening progressing to resistance bands,Glenohumeral internal rotation strength improving,No pain with functional reaching tasks at waist to shoulder height,Proprioceptive training demonstrating improved kinesthesia
→ Advance to: Advanced Strengthening Phase
Rotator cuff strength grade 4/5 in all directions,Scapular muscle endurance adequate on prone hold tests,Functional reaching above shoulder height without compensation,Shoulder flexion endurance approaching contralateral side,No provocation of AC joint with overhead activities
→ Advance to: Advanced Strengthening Phase
Rotator cuff strength grade 4+/5 or ≥90% contralateral side,Scapular power adequate for throwing progression if applicable,External rotation strength at 90° abduction ≥85% contralateral,Plyometric progression tolerated without instability symptoms,Biceps tendon loading without anterior pain or weakness
→ Advance to: Return to Activity Phase
Full pain-free ROM in all planes including cross-body adduction,Rotator cuff strength ≥90% contralateral side on dynamometry,Shoulder outcome scores meeting discharge criteria,Functional testing at required activity level without compensation,Patient education completed for maintenance program
→ Advance to: Discharge
