Recovery phases

Week 1Protection and Pain Control

Pain at rest ≤2/10 on VAS scale,Costovertebral joint tenderness decreased by ≥30%,Ability to maintain neutral thoracic spine posture for 10 minutes without pain,No painful clicking or catching with deep breathing,Lumbar and cervical compensation patterns minimized during breathing,Paraspinal muscle spasm reduced on palpation

→ Advance to: Early Mobility Phase

Week 2Early Mobility

Thoracic rotation range of motion ≥15° bilaterally,No increase in pain with seated thoracic flexion/extension,Rib cage expansion symmetry within 1cm measured at axillary line,Able to perform prone lying for 5 minutes without increased symptoms,Facet joint mobility improved with segmental mobilization,Pain-free coughing and sneezing

→ Advance to: Mobility and Stabilization Phase

Week 3Mobility and Stabilization

Thoracic rotation ≥25° bilaterally in seated position,Costovertebral joint mobility restored at affected levels,Ability to perform thoracic extension in prone (prone propping) without pain,Intervertebral disc tolerance to loaded positions (sitting <20 minutes),Erector spinae and multifidus activation with palpation feedback,No reproduced radicular symptoms with segmental loading

→ Advance to: Strengthening Phase - Early

Week 4Strengthening - Early

Thoracic extension strength 4/5 on manual muscle testing,Able to hold thoracic neutral during functional tasks for 10+ minutes,Rib cage dynamic stability during respiratory cycle,No provocation of facet joint symptoms with loaded extension,Periscapular endurance >20 seconds in prone Y-T-W holds,Tolerance to prone lying for 15 minutes

→ Advance to: Strengthening Phase - Advanced

Week 5Strengthening - Advanced

Thoracic rotation strength 4+/5 through available range,Erector spinae hold time >30 seconds in prone extension,Lumbar compensation absent during thoracic exercises,Functional tolerance to sitting >30 minutes,Rib cage mechanics normal during deep breathing under load,Multi-segmental spinal coordination without segmental dominance

→ Advance to: Endurance and Functional Phase

Week 6Endurance and Functional

Thoracic mobility within 80% of asymptomatic side,Sustained thoracic postural control during ADL for 1 hour,Endurance of thoracic paraspinals >45 seconds,No recurrence of costovertebral or facet joint irritability,Tolerance to combined loading (flexion + rotation + compression),Patient-reported function improvement ≥30% on ODI or NDI

→ Advance to: Return to Activity Preparation

Week 7Return to Activity Preparation

Full thoracic range of motion in all planes,Thoracic extension strength 5/5 or 4+/5,Able to perform work-specific tasks without pain escalation,No nighttime pain or sleep disturbance related to thoracic spine,Patient satisfaction ≥7/10 with current function,Demonstrates proper body mechanics during lifting simulation

→ Advance to: Return to Full Function

Week 8Return to Full Function

Thoracic ROM equal to contralateral side or pre-injury baseline,Strength 5/5 in all thoracic movements,Tolerance to full work/recreational activities,No functional limitations in activities of daily living,Patient discharged with independent home exercise program,Outcome measures meet minimal clinically important difference

Return to activity milestones

Pain-free basic activities of daily living

Week 2

Transversus abdominis and multifidus co-activation confirmed on cue

Light functional activities and conditioning

Week 3

Pain ≤ 3/10 during static postures and positional transitions

Moderate activity and sport-specific conditioning

Week 4

Lumbar spine flexion ≥ 50% of expected range; extension ≥ 50%

Full return to sport, occupation, and recreation

Week 8

Transversus abdominis and multifidus co-activation confirmed on cue
