Recovery phases
Pain level reduced to ≤3/10 on VAS at rest and ≤5/10 with activities requiring wrist extension,Ulnar nerve sensation intact bilaterally with negative Tinel's sign at cubital tunnel,No exacerbation of lateral or medial elbow pain with resisted wrist extension/flexion,Joint swelling decreased with normal humeroulnar and radiocapitellar contour,Pain-free passive range of motion achieved for elbow flexion 0-130° and forearm pronation/supination 70/80°,Tolerance of gentle soft tissue mobilization to extensor carpi radialis brevis origin
→ Advance to: Phase 2: Early Mobilization and Neuromuscular Activation
Pain ≤2/10 with activities of daily living and ≤4/10 with resisted wrist extension,Grip strength measured at ≥60% of contralateral side using dynamometry,No reproduction of ulnar nerve symptoms with elbow flexion beyond 90° for 5 minutes,Pain-free wrist and elbow active range of motion within functional limits,Ability to perform isometric contractions of wrist extensors and flexors for 10 seconds without pain,Tolerance of progressive resisted exercises using theraband at Level 1-2 Yellow
→ Advance to: Phase 3: Progressive Strengthening
Pain ≤1/10 with light functional activities and ≤3/10 with resisted movements,Grip strength ≥75% of contralateral side,Full pain-free range of motion in elbow flexion-extension and forearm rotation,No ulnar nerve symptoms with sustained elbow flexion or repetitive flexion-extension,Ability to perform eccentric wrist extensor contractions without pain flare,Tolerance of closed-chain loading through radiocapitellar joint up to 50% body weight
→ Advance to: Phase 4: Advanced Strengthening and Functional Training
Pain ≤1/10 with all activities including resisted exercises,Grip strength ≥90% of contralateral side,Full strength in flexor-pronator mass and extensor carpi radialis brevis,No joint line tenderness at humeroulnar articulation,Successful completion of sport-specific or occupation-specific activities without flare,Nirschl Phase Rating Scale improved to Phase 1-2
→ Advance to: Phase 5: Return to Activity and Maintenance
Pain-free full participation in pre-injury activities at full intensity,Grip strength 100% of contralateral side with no fatigue-related decline,Full endurance of wrist and elbow musculature without compensation patterns,No recurrent swelling of elbow joint or ulnar nerve symptoms,Patient-Rated Tennis Elbow Evaluation score ≤10 points,Ability to perform high-repetition functional tasks without symptom recurrence
→ Advance to: Discharge with home maintenance program
Complete resolution of lateral and medial epicondylar tenderness,Normal ulnar nerve conduction studies if baseline abnormalities present,Maintenance of strength and flexibility without supervision,Patient demonstrates proper technique for sport or occupational activities,No limitations in return to work or sport participation,Self-reported satisfaction ≥8/10 with function
→ Advance to: Complete
Return to activity milestones
Pain-free basic activities of daily living
Week 6Patient adherence to home exercise programme ≥ 5 days per week
Light functional activities and conditioning
Week 10Able to perform light overhead reaching for objects at eye level
Moderate activity and sport-specific conditioning
Week 13Grip strength ≥ 50% of unaffected side on dynamometry
Full return to sport, occupation, and recreation
Week 23Patient reports ≥ 30% improvement in upper limb functional score
Precautions
- warm-up-before
- avoid-overloading
Contraindications
- infection
- dvt-risk
- severe-osteoporosis
- limited-range-before-assessment
Pain management
- Ice application to lateral and medial epicondyle for 15-20 minutes post-exercise
- Non-steroidal anti-inflammatory medication as prescribed for acute inflammation
- Soft tissue mobilization to flexor-pronator mass and common extensor origin
- Ulnar nerve gliding exercises to prevent neural adherence
- Counterforce bracing at proximal forearm to reduce extensor origin stress
- Modalities: ultrasound or shockwave therapy for chronic tendinopathy
